Polka, Florence Myrtle Florence Myrtle Polka, age 84, of Phoenix, passed away on Tuesday, April 25, 2017 in Mesa, Arizona. She was born on November 2, 1932 to the late LaSelle and Nellie Bates in Phoenix. She graduated from Phoenix Union High School. Flo worked as an Executive Secretary at Motorola. She was active in the Church of Jesus Christ of Latter Day Saints and was an accomplished pianist and organist for the church. Flo was an avid reader and enjoyed spending time with family and friends. She is survived by her loving children, Douglas E. (Michele) Smith, Karen L. (Jeff) Tate, and Debbi (Robert) Felice; a sister, Dixie Wood; seven grandchildren; 16 great grandchildren; and many other loving family and friends. She was preceded in death by her husband, Frank Polka, and son, Jon Smith. Funeral services will be held at 10:00 am on Tuesday, May 2, 2017, at the LDS Thunderbird Chapel, 3010 E. Thunderbird Road, Phoenix, AZ 85032 with a viewing beginning at 9:00 am. Graveside services will follow at Greenwood Memory Lawn Cemetery, 2300 W. Van Buren, Phoenix, AZ 85009.
